At Physical Therapy Works, we want to provide you with answers to the questions we are most frequently asked. Before you arrive, you will be prepared, ready to start your recovery and get back to what is important; being you again.
What do I need to begin therapy? You will need a referral from your physician. Once you have a referral, call Physical Therapy Works at (904) 543-9011 to set up an appointment.
What insurance policies do you accept? Physical therapy Works is an in-network provider for ALL major managed health care plans. Should you have any questions regarding a specific plan, please contact us for further information at (904) 543-9011.
How long will my evaluation take? Evaluations range from 30 - 60 minutes depending upon the diagnosis and type of problem. Your therapist will discuss with you the expectations for your recovery and will help you set realistic goals and timelines to complete your recovery.
What should I wear to therapy? Loose-fitting, comfortable clothing and shoes are most appropriate for therapy sessions. In addition, make sure to bring any braces or splints that you have.
What do I need to bring to my therapy session? On your first day of therapy, remember to bring a photo ID, your insurance information, any paper referrals, insurance pre-authorization, prescriptions and any copies of diagnostic tests or procedures.
Medical History Before you arrive at Physical Therapy Works, please take the time to print out and complete our online Patient Information and Health Information forms, Consent forms, and if applicable, the Medicare Questionnaire below. This allows us to determine if there are any preconditions that may prevent you from being able to receive rehabilitation treatment from us.
